HDFS Peer Mentoring Program Participation Form                                                                                    
The Human Development & Family Studies Peer Mentoring program is a supportive community for new first-year HDFS students who identify as first-generation college students and/or students of color.  Participation in the program is interest based and is a full-year commitment.  

Program Support & Highlights Include:
-Mentoring & Community within your Major: In addition to campus mentoring programs, the HDFS Peer Mentoring program will help you build connections with your peers, your mentor, faculty, & staff in the HDFS major.

-Dedicated Peer Mentor: You will be paired with a junior or senior HDFS student who will be your support, friend, resource, & Peer Mentor throughout your first-year at CSU.

-Enrollment in 1-credit  "Mentees First-Year Success" course (HDFS 160). You & your mentor will take the course together. The class is in-person Wednesdays | 4:00pm - 5:40pm.  

-Program Events: Social & academic events are offered throughout the year to students in the mentoring community.

Active engagement in the HDFS Peer Mentoring program is expected from all students participating in the program.  Please confirm that you'll be able commit to the following:  (1) Commit to participating in the program for both fall and spring semesters.  (2) Register & attend Peer Mentoring class (1 credit each semester, 100-level)  Wednesdays 4:00-5:40pm. (3) Meet with mentor regularly outside of class *
